If you are interested in receiving mobile services, I will come to your house with any necessary ​equipment and evaluate or treat you from your home.

I am currently in-network with Blue Cross and Blue Shield, BCBS Blue Advantage, and Medicare Part B. I do offer self-pay rates for individuals who do not carry these insurances.

Yes, I can provide you with a superbill so that you may try to submit for reimbursement through your ​insurance company.

This is no guarantee, but some insurances may reimburse you if you have out of network benefits. Many times ​choosing to go the self-pay route ends up being cheaper in the long term for individuals with high deductible health plans.

Unfortunately not currently, but I hope that changes in the future! For my self-pay ​patients dry needling is included in the cost. For patients using insurance, there will be an additional $20 fee for dry needling.

All sessions are 60 minutes in length.

Absolutely, I offer convenient 30 minute self-pay visits ​specifically for dry needling. I can needle 1-2 body parts in this span of time. These 30 minute sessions cost $65.

I currently accept debit, credit, HSA cards, check, or cash.

I offer free 15-minute phone consultations to discuss and assess your needs to ​see if Core Revival is right for you.
